In my game for example Genoa got seperate peaced out in a war i called them in which was against milan and some other allies. This is kind of unbalanced as of now and still not correctly coded. Idk what counts as good relations, but alliances definitely work. Assuming you've picked the option to try and keep the italian states in the HRE, you need to beat the states you want to keep in the HRE in a war (you just need to win it, dont need to take any provinces) or have "good relations" with them. With emperor dlc, you have to wait for the imperial incident.
